2) 5-10 minutes prior to your scheduled appointment, click on the link of the provider you are seeing below.







Need to pay your bill?  We are working on online bill pay: for now, please call or pay us through paypal.  If you have trouble paying your bill, please download or call and request ahardship application or aCOVID-19 waiver/reduction applicationIf you download and print, you can return these requests for bill reduction or waiver to us via fax at 503-474-3601 or take a photo and text it (and your patient information) to us at 971-312-2292.
